When everything is finished, a member of our staff will lead you from the waiting area to the treatment area where they will evaluate your dental history, medical history, and any dental health concerns or issues.

There is a NEW PATIENT form that we will require you to complete so the doctors have all the necessary medical information. Some of the documents needed: A list of current prescriptions (if any) and any relevant medical history, such as illnesses or previous dental care or oral surgery. Insurance cards and claim forms. Dental X-rays (if taken within the last six months).Any referral slips or letters.
